Adequacy of prophylactic antibiotics utilization by the size of hospitals in Korea

Abstract
Object : To find out the differences of adequacy of prophylactic antibiotics utilization by the size and referral status of the hospitals.

Method : Based on the claims data of Health Insurance Review and Assessment Service of 2007, 18,076 cases were selected for eight surgical procedures including stomach surgery, colon surgery, cholecystectomy, total knee replacement, total hip replacement, heart surgery, hysterectomy, and Caesarian section. Information on antibiotics use and patient clinical characteristics was collected by chart review. Administration rate 1 hour prior to skin incision, administration rate of aminoglycoside, and administration rate of multiple antibiotics was measured. Comparison of each measure was done by referral status of hospitals.

Result : Administration rate 1 hour prior to skin incision was 91.4 % in hospitals with 3rd referral level, which was higher than the other hospitals. Aminoglycoside administration rate was 11.9 %, and multiple antibiotics administration rate was 38.6 % in 3rd referral level, which is lower than general hospitals.

Conclusion : Hospitals with higher referral level showed higher adequacy in prophylactic antibiotics use, compared to lower referral level.
